When it comes to hyaluronic acid injectables (more commonly known as fillers), we tend to focus on areas of volume loss such as the cheeks, under eyes, and lips. The chin is often ignored but can be a great candidate for filler augmentation.

The young woman seen in photos came to our Physician Assistant Shea for a non-surgical chin augmentation. Shea injected her with Restylane Defyne, which is FDA-approved for chin augmentation and correction. This subtle enhancement has huge impact, bringing her face into balance.

Eyelid surgery, or “blepharoplasty,” has become incredibly popular since the pandemic thanks to the “Zoom Boom,” and to increased mask requirements, which make the upper face and eyes a focal point during everyday conversation.

Blepharoplasty is intended to enhance the upper and/or lower lids, to create a more refreshed, youthful look. For the upper lid, the procedure involves making an incision in the natural crease of the eyelid, and then removing and reshaping the skin, muscle, and fat that deflate and droop with age and sun exposure.Blepharoplasty can also be performed on the lower eyelid. As people age, the tissue that holds the fat in place can loosen and the fat can protrude, giving the appearance of bags under the eyes. These bags can be repositioned and reshaped.

Does insurance cover cosmetic eye procedures?

Generally, cosmetic services are not covered by medical insurance. But in some cases, like the patient shown below, if an eyelid’s drooping is severe and affecting vision, upper eyelid surgery may be covered. Simple tests can determine whether the condition warrants insurance coverage.

Diagnosis: Ptosis

This patient presented with droopy eyelids, also called ptosis (toh-sis), that may occur due to trauma, age, or various medical disorders. Ptosis can affect one or both eyes, and can be present at birth or develop later in life.

This patient had ptosis repair and upper blepharoplasty. 
Depending on the severity of the condition, droopy upper eyelids can block or greatly reduce vision depending on how much it obstructs the pupil. This patient had ptosis repair (covered by insurance) at the same time as an upper blepharoplasty. This was followed by a CO2 laser treatment to the eye area.

If working towards a PhD at UC Irvine was not enough, Klebea was also planning her wedding in Brazil. Wedding photos would be memories for the rest of her life. She was hoping to improve the unsightly scar on her cheek, a painful reminder of a past skin cancer removal.

She was referred to Dr. Simon Madorsky for scar treatment. During the office visit, he noticed something more ominous on her nose. Two separate nasal lesions were biopsied, and both were found to be basal cell carcinomas. As cancer treatment was planned, he treated her scar with injections and a topical scar cream. The cheek scar improved in time for the wedding, but the subsequent skin cancer surgery loomed large in her mind.

The plan was to undergo Mohs surgery­—a process that gradually removes skin involved with cancer, layer by layer, while sparing the surrounding healthy tissue, all in the same day. Each layer removed is examined with a microscope to track the cancer cell invasion. This process is repeated until no more cancer cells are seen.

Dr. Jonathan Baron performed the Mohs surgery. It took six layers to achieve cure. “He kept taking more layers, kept coming back. It was deeper than expected. I felt like 70 percent of my nose was gone.” Indeed, a large portion of the nose was lost to cancer. “We had discussed types of reconstruction, but I knew we would have to wait until that day to know the options.” When Dr. Madorsky told Klebea that a forehead flap was required for reconstruction, she started to cry.

Three months later, after the nose was fully healed, Klebea was not only cured, but free to live her life—finish her PhD, and enjoy being a newlywed. The SCARS Center team helped Klebea look her best for the wedding day and for every anniversary to come.
